Describe what documentation needs updated or added: An issue template for crash reports.
Why is this documentation needed: To allow users to quickly lodge a crash report such that it doesn't get lost in the Discord backscroll.
What medium is best for this documentation: Issue template with minimum of extra details needed, since those details are included in a crash log.
