Cck-specific registration #12

As discussed in https://github.com/Cambridge-Community-Kitchen/cck-volunteer-web-app/pull/11, this follow-on PR removes the 'nickname' field from the 'person' table, opting instead to place it into an 'addl_info' field on the 'organization_person' table.

It also creates a cck-specific registration endpoint that insures that the 'nickname' field is unique, as this is no longer enforced by the database.
