is_deceased and radiation_boost are no longer true boolean fields so needed to update this in the schema validation so that validation worked properly
change bools in csv template to single_val
Update test data to be compatible with the new model (mostly removing Unknown and capitalizing anatomical site irradiated)

(This won't pass tests until https://github.com/CanDIG/katsu/pull/262 is merged to develop, it fixes Not available capitalization)
