Update entity table

Attached is the new entity-level table and also a new version of the entity-level/display order-table. I have also updated the metadata about Entity to include entity 430 with the correct label/description. @HuidongTian so this is what you need to do:

• I have stored the current version of the icd10 to entity table in icd10_to_entity_columns.csv in https://github.com/CancerRegistryOfNorway/NORDCAN/tree/master/specifications; save Siri's ICD10 - entity_2020-10-16.xlsx as a .csv with the same column names and formatting as icd10_to_entity_columns.csv and just replace icd10_to_entity_columns.csv in that folder

• same approach with Entity - Level - Display_order_2020-10-16.xlsx, save it as a csv to replace entity_usage_info.csv in https://github.com/CancerRegistryOfNorway/NORDCAN/tree/master/specifications. take care with the column names!

• after the replacements, get the latest version of package nordcancore and run data-raw/sysdata.R. make sure everything works using devtools::check(). if nothing broke, commit everything and push.

Due to a new column (entity_level_13) was introduced, "nordcan_columns.csv" was also updated with one more line:
