Create new entity for anal cancers, update table of ICD10-entity.

See #30 for the updated entity list. Entity 105 is added for anal cancers. Entity 400 is added for leukaemias. Entity 431-456 is removed from the specification as we should no longer group into these entities in NORDCAN.
