add metadata passed from R #25

add optional argument metadata to survival_statistics.ado

a new argument metadata could be added after "country(\"%s\")" in

https://github.com/CancerRegistryOfNorway/nordcansurvival/blob/master/R/survival_statistics.R

the metadata would be string containing, version of package etc

this will follow data to IARC ( a separate row on the metadata field)
