Add "Feedback" button to Challenges #199

It would be awesome to add a Feedback button to challenges (and potentially other places on the site) that pop up a short survey that asks users to rate the challenge. Some questions might be (definitely not all of these)

Were you able to complete this challenge? ()yes, ()no, ()partially What was the most challenging part of the challenge? Text response How difficult was the challenge? EZ-PZ Impossible (7 pt scale) Was this challenge fun? Boring! Very Fun! What lesson or lessons helped you prepare for this challenge? Is there anything else you would like to share?

Do you want this to be a google form as well? One that pops up? How would you like this information to be saved?
