msvc reorganized: 2 solutions sc-hsm-pcsc-vs2010 and sc-hsm-ctapi-vs2010 #1

with their own project files: sc-hsm-pcsc-* and sc-hsm-ctapi-*. All project files define WIN32, and all sc-hsm-ctapi project files define CTAPI. Minor include and external fixes.
