msvc reorganized #2

Only 3 files of pkcs11 require small changes for compiling. ultralite: added comments and changed ctn parameters to static Ctn. msvc: 2 solutions, sc-hsm-pcsc-vs2010 and sc-hsm-ctapi-vs2010 with their own project files.
