Deduce algorithm metadata enhancement

Expand the enrichment of the algorithm with patient names to include healthcare provider names. Both should be optional. Furthermore we could allow lookup list expansion with e.g. lists of all patient and staff names within an organization, as well as patient system (such as family) names.

Because deduce offers the option to work with initials and currently a simple parsing attempt happens for patient names, this can also be included as three options:

• use dedicated initials column; handy if the source data scheme already has initials.
• parse initials from patient/staff/networkList names
• don't include initials (if guessing initials would lead to wrong tags that negatively affect the readability of output reports).
