Add User Dialog Revisions

Main Dialog

• [x] remove organization elements from main window
  • [x] Phone
  • [x] Email
  • [x] Address
  • [x] Start Date
  • [x] require First, Last, and Organization before the OK button is activated
  • [x] Edit and Delete buttons should be disabled if nothing is selected in the organizations listbox

Add

• [ ] Add address to Organization dialog

Edit

• [x] Organization Phone and Email do not reload when Edit is selected.
• [x] Accept and Cancel buttons have no effect
