Prepare for production (fix critical and non-critical bugs)

• fix issue where muting/pausing video by default doesn't work
• fix issue where cssglobalvars w/ theming breaks some builds
• fix issue with muting not working in some situations
• display indicator for when someone is muted/paused
• update chat UI
• fix bug with browser not resizing
• fix issue where catalyst would ask for perms even when already in call
• add handlers for arbitrary data
• trigger start call when at least one peer has joined for the first time rather than when object is instatiated
