What is a sleep diary? The sleep diary will help you to record your sleep behaviour.

How often do I need to fill it in? The diary should be filled in every morning for about 1-2 weeks. You also have the option to fill in your daily pain.

Is it mandatory to fill in the pain diary as well? No it is optional for you to fill this in every day.

What should I do if I miss to fill it in one day? Even though it is best to fill it in every day, it can happen that you might miss a day. In that case please continue to fill it in for the current day/past night and leave the missed day out.

What if there is something unusual that effects my sleep? For this you will have a page for you to fill in if there is anything else you would like to tell us .

Where can I find more information on the study and the Sleep Diary App? There will be a information page that explains different parts of the study.

Which questions will I find?: Each question will ask about your sleep behaviour either from last night or the morning after awakening.

1. On a scale of 1 to 9, indicate how sleepy you are feeling? Please tap on the number. 1 = very alert to 9 = very sleepy, great effort to keep awake to indicate your sleepiness.

2. How would you rate the quality of your sleep? The answers ranges form very poor to very good. There are some little emoji faces help you make a decision on how you feel.

3. What did you do in the HOUR before getting into bed? Here you can choose from different activities, such as: reading, Internet, TV and music. However, you have the choice to tell us something else that you did by tapping the “Other” button OR you can choose “I prefer not to say”.

4.What time did you go to bed? Please chose the time that you got into your bed to go to sleep.

5. When you got into bed, what did you do to try to go to sleep? Here you can choose from different activities, such as: reading, Internet, TV and music. However, you have the choice to tell us something else that you did by tapping the “Other” button OR you can choose “I prefer not to say”.

6. How long did it take you to fall asleep? Please indicate how long it took you to fall asleep. If you are not sure please estimate the time that you think it took you to fall asleep.

7. If you had difficulty falling asleep, tap the boxes to specify why? Here you can choose from different topics, such as: pain, noise and worries. There are some little emoji faces help you make a decision. However, you have the choice to tell us something else that you did by tapping the “Other” button OR you can choose “I had no problems”.

8.What time did you wake up? Please indicate the time you woke up in the morning.

9. How long did it take you to get out of bed in the morning? Please indicate the time how long it took you to get out of your bed in the morning after you woke up from your sleep.

10. Did you have frequent awakenings during the night that made it difficult to get back to sleep? Please chose yes or no. When choosing yes you can specify what made it difficult for you to go back to sleep.

11. Did you take any medications before sleeping? Please chose yes or no. When choosing yes you can specify which medication you took.

12. Is there anything else that you would like to tell us that could have affected your sleep? Please chose yes or no. When choosing yes you can tell us more on what could have affected your sleep last night.

13. Before going to sleep, how many sugary and/or caffeinated drinks did you have? Indicate the number below? Please indicate the number of drinks (e.g., caffeine or soda/energy drink) that you had before going to sleep. You can also chose “None of the above”.

14. Did you take a nap during the day? Please chose yes or no. When choosing yes, you can tell us the number of your nap(s) and the time you spend napping.

At the end of the Sleep Diary you are able to fill in the Pain dairy right away OR you can fill it in later throughout the day.
