Restore `inputUpAxis` and `outputUpAxis` axis #211

This pull request restores the feature that allowed users to specify the OBJ up axis. See #63 and #209

@FreakTheMighty this works great. I just pushed one doc edit (https://github.com/AnalyticalGraphicsInc/obj2gltf/pull/211/commits/fd6e98688d4b03083d67906de374faa767738c50), but the rest is perfect.
