Product not configurable #10

What steps will reproduce the problem?
1. client created an order and paid.
2. admin accepted order (certificate was purchased in sandbox)
3. client got "SSL Certificate Configuration Required" email
4. configuration link is clicked

What is the expected output? What do you see instead?
a configuration page is expected.
instead I see a table with 3 rows and nothing configurable (see attached image)

What version of the product are you using? On what operating system?
WHMCS: 4.5.2
NAMECHEAPSSL: SSL Module for WHMCS (1.1.4) 
OS: linux/apache

Please provide any additional information below.
