Draft of the Lead Safe application form #15

Have a brief description of the purpose of the form
Included all the necessary fields required for the application
Some validation is in place:
- Email addresses will be validated for proper syntax
- NPI is limited to 10 characters
- IPL is limited to 9 characters

Once #1 is created and finished, need to update links to the terms of service and eligibility requirements. I've noted this in #2.
May want to improve the look of the dropdown box (the select input).
The subject line is "Test Submission: Lead Safe API Request". This should remove "Test Submission" when it's no longer in testing.
