We need to clarify expectations for risk_score and risk_score_notes in the API documentation. The format descriptions are accurate, but we are still using the basic descriptions that were drafted awhile ago.
We should be more specific to describe the expected contents of risk_score_notes and that risk_score will be a risk ratio (see Chicago/lead-model#9).
