Eliminate `visit_date` from prediction? #7

I don't think this belongs here. The timestamp is sufficient to communicate the temporal aspect of the score. We have no knowledge of when the visit is going to happen. We're not even getting visit_date in the POST for the current visit (we do get past dates of visit, but even those are not considered "visit dates" per se).

I can't think of a reason that it belongs here, but I could be forgetting / overlooking something.

we are keeping this and updating the description to indicate that it is a "vintage date" for the model, and this date will be optional

Note this field is not going to be optional.
