Form action changed to "/submit.html". Fixes #26 #27

Previous action was set to the root page (index.html). When the submit button was pressed the browser would take the user back to the index page without submitting form data - because of this issues were not saving properly.

I've read this commit but I'm wondering what exact line change in the commit mentioned in this issue commit: f24bca2 If you could add that it would be great for future bugs of the same nature
