I want to stack multiple radio buttons/slider bars on one page

As an editor, I want to create an item in an activity that has multiple slider bars so that I can receive input from the user on one page

Requirements:

• Provide a way for the editor to have multiple slider bars on the same page
• Each slider bar would be associated with a different item and act independently
• Possibly create this or radio buttons and checklists as well

Based on @minji5924's work on the Pediatric Screener and some feedback we've gotten from the clinicians regarding the DBT Daily Diary card, we might want to consider expanding this tabular layout to radio buttons, checklists, and slider bars.

Here's what these widgets might look like:

Applet Builder - Multiple Slider

• Create a new item type called Stacked Slider
• Add an option to add another slider to the slider item
• Allow the editor to set the min/max value and min/ max label
• Allow the user to add images to min/ max ends of each slider added
• When an editor adds a slider label, the text shows to the left of the slider like in the example "Happiness
• If an editor wants to set the scores then show each slider stacked on the screen with the ability for the user to enter the numerical scores
• Allow the user to add, edit and delete entries in either list
• Allow the editor to set scores for each response option
• Allow the editor to select if the item is required (skippable)
• Tick marks, and continuous slider are not in scope for this ticket

Applet Builder - Multiple Choice/ Radio Button

• Create a new item type called Stacked Radio Button and Stacked Multiple Choice
• Response options are the X axis
• Item list entries is the Y axis
• Allow the editor to add images to the item list
• Allow the user to add, edit and delete entries in either list
• Allow the editor to set scores and tokens for each response option
• Allow the editor to select if the item is required (skippable)

Works as expected only for stacked sliders on the app side.

Preconditions: The applet with staked radio/checkboxes are created on the admin and uploaded Failed for the stacked radio/checkboxes.
