Frontend: Show a (customized) consent form when a user signs up for Mindlogger. @akeshavan

@shnizzedy I think we should have a general consent form/terms & conditions for signing up for Mindlogger itself, and then a per-applet consent form. What do you think?

Completely agree. Mindlogger and its content are separable things, and we should treat them as such

[ ] add a Mindlogger terms and conditions section to the signup template
[ ] add a consent form key to the schema at the acitivityset/applet level https://github.com/ReproNim/schema-standardization/issues/41
[ ] show the consent form if the user has not consented for a given activity set
[ ] add a server route to store whether a user has consented to an activity set (https://github.com/ChildMindInstitute/mindlogger-web/issues/11)
