This is a weird issue that's been going on for almost 4 years now and I just decided to report it. I thought the problem was being caused by the 81 tiles mod, but I've been playing a game without it and it still appears to happen. Transit lines will break and stations will re-appear somewhere else on the map.
Steps to reproduce
I honestly do not know a good way to reproduce this, it just randomly happens. Regardless of what mass transit line you use, it can be buses, monorail, subway, etc. after hours of play the game will relocate stops along a set line. When this happens, there will be a break in the line (see my screenshots) and then the line will restart somewhere else on the map typically with 2 orphaned stops. In the example screenshots this was a monorail that suddenly broke and then restarted somewhere else.
I don't know if this is your mods issue. It's so hard to replicate and I can't replicate it myself within the game. It just happens. Sometimes I will get an error in the game and I've reported that behavior under someone else's reported issue, so maybe once I resolve the pop-up error by clearing all traffic the lines are broken? I will need to check next time to see if that's what is happening.
