Extension -> Namespace #10

Description

Rename all extension references to namespace. According to the spec it is called namespace.

After having another look into the specification, I think extension may actually be appropriate as the specs speak of "context extensions" which do not necessarily have to be namespaces (see here). Maybe, however, we could rename the references to context_extension to make this more clear?
