Media names are garballed #4

The media name that's posted (and shows up in CRM) is a bunch of gibberesh characters. If we don't have a good filename from the user, the open311-nodejs client should invent a filename that looks nice.

This issue is probably moot, once we never use user-provided filenames at all.

The media filenames are no longer used in the CRM server.
