Indicator type box has disappeared from the user interface #2

I added a few operational indicators for action 144, and strange things happen. 1) The indicators do NOT show up on the page of action 144. But they DO show up in the insight network. 2) The button to choose indicator type (operational, tactical, strategic) has disappeared. Hence, the indicators show up as grey circles on the network rather than green boxes.

Also, now that there are several action plans under monitoring, the action plan should be asked when adding actions or indicators? Interestingly, when I added operational indicators for HNH/144, they show up in the insight network, but when I added a tactical indicator that is pointed to by these operational indicators, the tactical indicator does NOT show up in the insight network. Does it need a direct connection to an action in a particular action plan?
