The Assessing team needs the form to continue this "for office use only" section

We could deliver this using the SeamlessDocs feature (Draft 2) but that would be the only SeamlessDoc in the entire City. That's a UI experience that is completely unique (not ideal) as well as a functionality we can't commit to support in future versions of a forms tool (which would presumably have digital workflows, but still).
