Want to update this for 2018-2019. Didn't love last years, but going to try making a new draft to see how that works.

Made a bunch of tweaks but the core of this is good. There is some discussion about how to handle recommendations.

Related to #431 -- Some recommenders are nervous having their letter transmitted via the student, so we are creating a separate form for them to submit themselves.
