Change app header to use absolute link to app url #68

Link in application header (at url /apps/[id]) should be absolute and point to the appsite, nor relative to civicstack own domain.

Disregard the pull request. The approach is too simplistic and it won't work in the case in which protocol (http://) is already included as part of website attribute.
