Create tutorial notebook for Quantum Entanglement #106

Created a tutorial notebook which closes #28 for Unitary Hack. The notebook includes a walkthrough of setting up a Bell state using Hadamard and Controlled NOT gates. It also contains a mathematical description of the code modeled after the superposition tutorial notebook's format.
