I think something like chronic back pain that scales with the load you are carrying or something like osteoarthritis that scales pain to activity level or maybe generates pain based on movement type.
all pain does is decrease your stats and some speed; while it is possible to make this in like few lines of code, i don't really see is there a reason

I am always frustrated when I fight something in melee combat and don't die incredibly quickly, and also when I am able to just go to sleep without issue or even effectively focus on a task.

As an alternative to behaving like this, there should be a Chronic Pain trait that adds a level of background pain to everything (minimal or moderate depending on how debilitating it should be). It would be independent of and stack with any injuries, punishing getting hurt hard. Since this would be pretty awful to play with, I think earning 4 or so points from it would be pretty fair.

Additional context
Now I can roleplay as an elderly or disabled person and really feel how hard it is to get literally anything done with a broken body. This would also just be generally nice for an increase in difficulty, especially for people like me who rely on speed a lot and don't plan escape routes well enough.
