Code changes required to respond to reviewers

[x] Improve adaptation model (following DeBruin et al 2009's AD-DICE model)
[x] Update adaptation costs (currently poorly justified)
[x] Update climate damages (and clarify that they represent gross damages while in DICE they are residual damages + adaptation costs)
[x] Update mitigation costs (too low right now)
