Add Contributor Affiliations #2246

Need to add in two new affiliations for external contributors. They will have no members, no rights to curate nor publish, etc... but they will have a name and an affiliation ID, and appear in the pulldown in #2179

Add two dummy affiliations (10084 & 10085)... see Affiliations spreadsheet. These dummy affiliations will never have any members but are required for tracking work added by these affiliations by others, such as for Contributors.
