What steps will reproduce the problem?
  "set vb" in .gvimrc or .vimrc

What is the expected output? What do you see instead?
  I should get a visual bell.  Instead, I just disable the audible bells.

What version of MacVim and OS X are you using (see "MacVim->About MacVim"
and  "Apple Menu->About This Mac" menu items, e.g. "Snapshot 40, 10.5.6
Intel")?
  Snapshot 66, 10.8.2 Intel

Please provide any additional information below.

It has not been implemented.  I have no particular interest in this feature so 
I'll leave it open for somebody else to patch.

You can set visual bell system-wide in Preferences/Accessibility/Sound and turn 
down alert sounds volume.
