When you sneak, you start taking damage. Then you die. However, if you are standing on a transparent block, like leaves, you don't take damage. But any solid block like dirt, sand, cobblestone, you will take damage.
You also take damage from swimming too long in the water, but I'm not sure if that one is a feature or if the two are somehow related.
