Add playbook for troubleshooting #221

We should create a document describing common issues that may come up and how to fix/troubleshoot.

e.g.

Issues

Trip times are missing or stale

The CSV published by HRT might be stale. Email (point of contact) and ask them to check the server that publishes the CSV.
