What does this PR do? Solves https://github.com/CodeForAfricaLabs/HealthTools.API/issues/68 and https://github.com/CodeForAfricaLabs/HealthTools.API/issues/57.

Combined the tasks into one since they seem interwoven.

Why are we moving the manage.py file? healthtools/manage.py → manage.py
