More form toggling options #396

The goal is the give users multiple options to toggle between forms.

1.

add a checkbox to the participant search. the label should be "toggle to SEP form on search" (unless you can think of a more succinct phrase).
and conditional statement that toggles the form, after the response is received from the server.
do not submit the checkmark value as a param

duplicate the dropdown from the top of the sidebar at the top of the participant table.
- make sure they remain in sync
