added an example contribution guideline #170

Hi all, I threatened to create a contribution guideline, and so I have. Max and I have been talking about workflows, and this guide specifies one. I'd like to make sure people agree with the model that is proposed ( fork and pull-request as the principal mode of contribution, except for 'contributors' who will be able to review the requests, or push directly to the hosting project ).

There are definitely places for improvement, esp around testing and code style, and also where to go if the is a question that the document doesn't address.

oh yeah, also, I totally disregarded my own guidelines inasmuch as i did not create a branch for this change because it was a single .md file addition.
