Remove Profile -- triggers legacy Heroku build pack, which defaults to dev mode
use fastify-cli, not much difference here, but wanted to try it for npm create fastify structure, looks like might help manage the project going forward
includes index->app changes
loads wasn't even installed in web, how heroku build was running it -- nobody knows, install only needed functions explicitly
