Attachments don't seem to be supported

Describe the bug

There are two avenues for attaching attachments (PDFs, jpegs, ...etc) when submitting a UIPA request:

1. When initially creating the request
2. When posting updates to the request thread

Expected behavior File attachments are supported in both cases.
