Update transport narrative

@rsingh2507 - overall we need to have each section getting down to HL7 segment detail by examples and bring clarity to the following:

• Order placer and order filler numbers relationship between ORM / ORU and FHIR attributes should be documented.
• In case of polling FHIR, the search by order placer and order filler numbers should be supported and documented as examples.
• Any literal values for the OBX segment, like the LOINC code for genomic lab result that will be used to described the FHIR API URL should be documented.

Recommend including Snippets of relevant HL7 ORM/ORU in each section
