Define feedback format for PD volunteers and trainees

Background Create a simple process so people can give us feedback on the content. One option is to create a Template Issue, as the Coursework PD Template, for people to raise it

Acceptance criteria

• Process needs to be simple
• Infomation needs to be minimal
• Ideally, the person raising it can already give suggestions about what should be changed, so we can copy and paste it
• We need to understand the WHY people are raising it

OK, I'll try and design a form which is simple but sufficient and not too time consuming.

Just create the example/fields. I'll create the template later, it's easy to do on our repo. I can show it to you.

Changes

• Title to be as specific as possible in 1 phrase. (Example: Difficult workplace conversations - Week 2 - React - Prep)
• Remove Syllabus Topic and Selection in the week
• Bring URL and copy of text to the top
• Add Other to the suggestion type
• Add Trainee/Graduate on the Role, Remove Other
• More detail on your feedback to be part of the "Suggested content"
