Description

Page was a bit long, so I tried to reduce some content and link to the correct pages, as well as reduce the number of words. I corrected the link to the new form. ## Who needs to know about this? @MarianaVazquezCYF @AliciaE08 @ShaunCYFSA @martaesn

This PR had a conflict in it @kfklein15 so it wasn't merged. I've resolved the conflict and approved. I've tagged @esma-g and she can review and you can go ahead and merge.
