Allow O13 accounts to add parent contact information #1151

As it is now, an O13 can sign up to their own account, and join and attend a Dojo by themselves. As a result though, Dojos don't have any parent/guardian contact information for emergencies. We should add the ability to add parent/guardian contact info to a standalone O13 page.

Should this ability also be able to be enforced/required depending on the Dojo? Could be a popup at join which says 'this Dojo requires more elements of your profile to be completed' or something.

This info is not related to a dojo itself, so that it should probably be shared upon multiple dojos. We should let it be inputted on the profile and have a dedicated "on-join" page when you don't have it filled. The main problem is : what is a decent "information"? Considering o13 don't require validation upon joining, you could fill it with "blablabla" and still be useless to the dojo. Should it be a mandatory phone number ? An address? A name?

Fields; Emergency contact information Parent/guardian name Parent/guardian email Parent/guardian phone number

Or we could get them to fill out only name and email then send an email to the parent to fill out all the information as this avoids them entering incorrect information?

Also it is related to the Dojo as some will require this and some won't but it's best practice for the Dojo to have this information.
