adding signup page #37

Added signup page including the form, formatting and related CSS. Adjusted the previous form that was in there to align with login page.

when creating the pull request I forgot to change the "commit into" branch to signup-pg and it defaulted to master. I changed this so it should be okay now but please double check before confirming changes. Thanks!
