Uniswap V3 fixes #109

I fixed a bunch of small, annoying bugs:

wrong Kovan DAI address
use fees.js for gas calculation globally instead of ethereum-gas.ts (we should resolve this duplication later)
fix wrong constants used in various files
change default gas limits for Approve and Uniswap V3
increase price precision
