PDF files usually open as read-only, which explains the reason users are unable to insert images.
Thank you for the info - I adjusted this issue as a feature request to add inline PDF annotations (in PDF anchored to page positions) and images. Would be a GREAT feature to have CODE as the central office solution for daily activities - especially as CODE already supports viewing PDFs.
Ultimately it would be great to have a bunch of features shown off as in https://www.pdfannotator.com/en/how like using a Text Marker in different colors, be able to use a Freehand annotations ans have some stamp and ability to add text and signatures.

I am using CODE in Nextcloud. When oping a PDF in CODE I see a menu to annotate, or add image to the PDF.
Adding image menu is greyed out. It would be highly appreciated if adding a PNG or SVG with the possibility to resize and position the image in the PDF would be possible. Placing a scanned signature could be an important use case. Optional: Drawing the signature with a pen like Apple Pencil.
