Better TOC extraction, add `fill-unique-refs` operation #4

The latter is an operation that goes through a document, giving each section a unique reference ID (The slug of its title, and if two sections have the same slug, appends an integer). This is used by TOC extraction to ensure every section has a valid link to it.
