Adding new fields in XForms copies multiple items from the templates #29

Click "add subject" or similar; multiple subjects will be added, because the 
XForms action is copying from the template instance using an XPath query that 
selects more than one subject. Fix by changing the XPath to return a single 
element, e.g. by adding "[1]" onto the end.
