Using the Cosmetic Settings Clothing panel and switching the Main Hand from right to left only puts the primary weapon (in this case a sword) in the offhand, making it unusable. It must be either switched back, or put back in the toolbar in order to use it again. Placing it in the toolbar and using the weapon this way puts the weapon back in the (right) default main hand.
Disclaimer: This may be a server issue, as this issue can not be replicated in a single-player session.
STEPS TO REPRODUCE:
Open "Cosmetica Settings..." under Config/Settings.
Click on Clothes, and view the next set of options.
Notice the Main Hand selection as right, by default.
With weapon equipped in the toolbar, switch the Main Hand to left.
View inventory and notice it is placed in the Offhand box (and can not be used).
EXPECTED BEHAVIOUR:
Switching Main Hand to work properly as it is featured in vanilla Minecraft.
(additionally, switching the main Offhand)
